Modified Kuhnt-Szymanowski Procedure for Lagophthalmos due to Ectropion

Abstract
Purpose: To evaluate the effect of modified Kuhnt-Szymanowski operation for lagophthalmos due to ectropion.

Methods: Eleven patients twelve eyes with lagophthalmos due to ectropion were treated using modified Kuhnt-Szymanowski procedure from August 2004 to August 2005. After incision along the marking drawn below cilia of lateral part of lower lid, tarsal plate and cutaneous muscle layer were dissected. After excision of lateral tarsal plate at 1 mm apart from lateral canthus, pentagonal excision of tarsal plate was made along full length of tarsus. After remnant tarsus was sutured at 3 sites with Ethilon 6-0, redundant skin was excised and skin sutures were done.

Results: The subjects was eleven patients twelve eyes. The mean age was 69.8¡¾10.1 years. The mean follow-up period was 6.1¡¾5.5 months. The causes of ectropion include 10 cases of facial palsy with the mean affected period of 20 years and 1 case of post-lower blepharoplasty complication. In all cases, symptoms of ectropion and lagophthalmos were improved without complications such as inflammation or ectropion, etc.

Conclusions: Modified Kuhnt-Szymanowski procedure for lagophthalmos due to ectropion is safe, simple, less time consuming and effective method with high cosmetic satisfaction.
